Hours and Appointment Times:
Appointments are 45-60 minutes in length and are scheduled individually each week. My regular office hours are as follows:
- Mondays 10-7
- Tuesdays 8-5
- Wednesdays 8-5
- Thursdays 8-5
- Fridays 8-5
Professional Fees:
- Initial Intake Session(s) (1-2 sessions as necessary) (60 minutes) $200.00 each
- Individual Session (45 minutes) $150.00
- Individual Session (60 minutes) $175.00
- Family Session (45-60 minutes) $175.00
- Missed appointments with no notice of cancellation - full fee of session (see above)
- Less than 24 hour notice for cancellation $50.00 (I always consider broken appointments individually and understand that emergencies do arise)
- Returned check fee/NSF Credit Card fee $ 25.00
- Phone call/consultation lasting over 10 minutes will be charged at $25 for every 1-15 minute segment after the initial 10 minute period. (If an extended discussion is needed I encourage you to make an appointment. There are occasional circumstances when it makes sense to have a phone session, however, please be aware that insurance companies do not cover this option)
- Court appearances $250.00 an hour (travel time, wait time, and court time will be billed, if court is canceled a minimum of 2 hours will be billed)
- Request for letter writing or completion of paperwork such as FMLA or disability assessments, starts at $25.00 (depends on nature of request and time required to complete the request)
- Copies will be charged at 25 cents per page.
Insurance
I participate with most major health insurance plans and am willing to bill accordingly. Services may be covered in full or in part by your health insurance or employee benefit plan. Balances such as deductibles, co-insurance and co-pays are due at the time of session and a credit/debit/ or FLEX card is requested to maintain on file. I encourage you to contact your insurance company to verify your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Do I have a co-pay or co-insurance amount to pay?
- Is approval required from my primary care physician?
